Use the Excel version online if you want the form to auto-calculate your expenses.

- Complete shaded areas of form online. Use the TAB key to advance.
- Print the completed form.
- Obtain signature from your Unit Director (include unit financial support and number of release days).
- Mail completed form to the Chair of the Staff Funding Committee.
